The registration form is to be completed and returned to the clinic with a copy of relevant medical records such as blood results and semen analysis.

Visa application letter

Upon scheduling the first appointment a visa application letter must be requested. If you require a visa for travelling to South Africa, a Medical Treatment Visa is recommended in lieu of a visitor’s visa that cannot be easily extended.

It will also include the estimated costs of the treatment and the period of intended treatment. Your full names, passport numbers and passport expiry dates (for both husband and wife or partner/s) are required.
